Additional Details:
Adult annual visit includes full physical exam and recommended lab work along with subsequent telehealth or in-person review of labs.
Must be 18+ to qualify as adult for annual exam, labs, b12 injections, weight management consult, and IV infusions as noted in membership plans.
Included sick visits apply to both in-clinic and telehealth visit. Service fee added for in-home visits at the core level. Waived for premium and executive when within published radius of service. Availability may vary for in-home requests.
Management of chronic on-going medical conditions and on-going prescriptions included at executive level but may be added to core or premium plans at reduced cost.
Direct communication with provider provided at executive level during extended hours of operation. Guaranteed response within 4 operational hours.

$150/month. 12-month minimum commitment.
